This Privacy Policy explains how Augustine Home Health Care handles information when you visit ahhc.org, contact us, or receive our in-home care services.
We do not sell your personal information. We do not use client information for advertising.

- Schedule and deliver home care services

Augustine Home Health Care is required by law to protect the privacy of your health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

We may use and disclose protected health information about you for treatment, payment, and health care operations — for example, coordinating your care with caregivers and clinicians, billing for services, and quality and training activities. Complaints
If you believe your privacy rights have been violated, contact our Privacy Officer (below). You may also file a written complaint with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

We may update this Privacy Policy and HIPAA notice from time to time. The current version will be posted on ahhc.org and is effective when posted. You may request a copy at any time.
