This Privacy Policy explains how Augustine Home Health Care handles information when you visit ahhc.org, contact us, or receive our in-home care services.

We do not sell your personal information. We do not use client information for advertising.

  • Care-related information needed to evaluate and provide home care (for clients and authorized family contacts)

  • Respond to consultation requests, questions, and applications
  • Schedule and deliver home care services

  • Operate and improve ahhc.org

Augustine Home Health Care is required by law to protect the privacy of your health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose health information

We may use and disclose protected health information about you for treatment, payment, and health care operations — for example, coordinating your care with caregivers and clinicians, billing for services, and quality and training activities. State law may require consent in some situations; we obtain consent when required.

With your agreement (including verbal agreement where appropriate), we may share relevant health information with a family member or other person you identify as involved in your care or payment for that care.

For uses and disclosures not related to treatment, payment, or operations — such as certain marketing or research uses — we will ask for a separate written authorization when the law requires it. You may revoke an authorization at any time for future uses and disclosures.

We may also use or disclose health information without authorization when required or permitted by law — for example, public health reporting, abuse or neglect situations, health oversight, court orders, law enforcement, or other purposes allowed under HIPAA.

Complaints

If you believe your privacy rights have been violated, contact our Privacy Officer (below). You may also file a written complaint with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

Changes to this notice

We may update this Privacy Policy and HIPAA notice from time to time. The current version will be posted on ahhc.org and is effective when posted. You may request a copy at any time.
